Leasing Forms for Framingham Housing Authority Tenants and Owners

Please note the attached forms must be completed and returned to our office to begin the inspection and leasing process (please note this process takes a approximately two weeks):

Request for Tenancy Approval (printable PDF) fully completed and signed by both owner and tenant.
W-9 Tax Id form (PDF) must be completed by the owner and signed and dated. (Owner must complete for year-end 1099 statement)
► Proof of ownership (A deed or recent real estate tax statement matching the W-9)
Electronic Funds Transfer form (PDF) (All owners must sign up for direct deposit)
Disclosure of information on lead based paint (PDF) (owner/agent and tenant must complete form)

Lead Certificate: If the family has a child under the age of 6 and the unit was built before 1978 then the owner must provide a lead certificate stating the unit is free of lead based paint. The owner must provide the form along with the disclosure of information on lead based paint.

Please note: To ensure timely payments from Framingham Housing Authority; the owner or tenant must provide Framingham Housing Authority with copy of the Lease agreement and original copy of the Housing Payment Contract before the 24th of the month. We must ensure that the information including the lease start and expiration dates on the lease matches our Housing Assistance Payment (HAP) Contract. We must also ensure that the lease does not violate any Federal Regulations or HCVP Policies. Framingham Housing will not make any Housing Payments on the tenant’s behalf if they move in to the unit prior to our full approval.
