An Option Counselor or Information and Referral Specialist will be available to answer any questions regarding long term care services and supports as well as resources to improve your health and well-being while living in the community setting of your choice.

Resources and information can be provided regarding an array of services aimed at helping people live as independently as possible, meeting medical, social, financial and housing needs.
